How accurate are these numbers?

The maths is exact: volume × fee rate × cashback rate. What varies in the real world is your order mix — every limit order that fills as maker costs you less than the taker rate used here, so the calculator is deliberately conservative on fees and on the cashback that follows from them.

Should I calculate with spot or futures rates?

Futures, if that is where most of your volume is, because taker rates there are what most active traders actually pay. Spot fees are typically higher per trade but the volume is usually lower. The cashback percentage is identical either way.

Do I have to sign up to use this?

The calculator infers a tier from volume alone, but most exchanges also grant tiers on asset balance. If your real tier is higher, your fee is lower — and so is the cashback in absolute terms, because it is a percentage of what you actually paid. Your effective rate still drops by the same percentage.
